Every caregiver starts with a prescreen to identify their level of stress and caregiving burnout risk. Based on the results, they may be connected with a Family Caregiver Support Program Social Worker who uses the TCARE system to create a tailored care plan to support their caregiving journey. The program offers caregiver training, support groups, home adaptations and safety devices, caregiver resource library and computer center, information and assistance, public information and community education for eligible caregivers. It also helps caregivers with in-home respite and out-of-home respite (day care) for caregivers at risk of caregiver burnout.
Family Caregiver Resource Centers
Local Family Caregiver Resource Centers provide assessment and case management for family caregivers in Ventura County. This program offers caregiver training, support groups, home adaptations and safety devices, caregiver resource library and computer center, information and assistance, public information and community education for eligible family caregivers. It also helps caregivers with in-home respite and out-of-home respite (day care) for caregivers at risk of caregiver burnout. See the list below for a program site nearest you.

FCRC Senior Concerns – Headquartered in Thousand Oaks and serves east Ventura County, i.e., Thousand Oaks, Simi Valley, Moorpark, the portion of Westlake Village in Ventura County, and unincorporated areas of east Ventura County including Newbury Park. VCAAA’s FCRC Santa Clara Valley – Headquartered in the Santa Clara Valley, La Buena Vida serves all caregivers in West Ventura County, including Santa Paula, Fillmore, Piru, Ojai, Ventura, and Oxnard. Trualta
Trualta is an online tool for managing care at home provided by the Ventura County Area Agency on Aging. Explore options for challenging behaviors and discover ways to further connect with your loved one. Connect, share, and learn with fellow caregivers.
WeCare
WeCare offers caregiver training videos in English and Spanish to unpaid caregivers to strengthen their caregiving skills and knowledge, and to receive ongoing support. WeCare is a web-based application that is easy to use and offers short training videos and resources that will help you increase your knowledge and skills based on your individual caregiving needs.
